Quillen College of Medicine Internal Medicine cares about its residents, fellows, faculty and staff. We have created several forms below so that we can recognize important exciting events and engagement. We also want to support our trainees during times of growth and learning with forms to recognize concerns about them. Please help us stay in touch!

Use the Internal Medicine Kudos form to recognize someone for going over and beyond
expectations. They will be recognized in the newsletter, with a kudo and be part of

GENERAL SUGGESTION BOX
I have a general suggestion for the Department of Internal Medicine.
CONCERN
Professionalism Concern - Internal Medicine is dedicated to helping physicians, faculty and staff to develop
and maintain professional behaviors. Professionalism may encompass communications
between patients and providers; appropriate attire/appearance; reliability, motivation,
and responsibility; interpersonal relationships; ability to accept instruction and
feedback; integrity.
Through modeling and upholding professionalism, Internal Medicine supports ETSU’s values statement: people come first, are treated with dignity and respect, and are encouraged to achieve their full potential; relationships are built on honesty, integrity, and trust; diversity of people and thought is respected; excellence is achieved through teamwork, leadership, creativity, and a strong work ethic; efficiency is achieved through wise use of human and financial resources; and commitment to intellectual achievement is embraced.
The Faculty, Staff, Resident and Fellow Professionalism Report is designed to allow faculty, staff, fellows, residents or students to document an incident or trend of unprofessional or unethical behavior. It is also used to measure institutional success in accomplishing the Internal Medicine and its subspecialties Professionalism milestones.
Every effort will be made to address each issue while maintaining confidentiality for all parties involved.

CARE Form
A Care form is an online form that anyone associated with Quillen can complete to
call attention to concerns regarding a resident or fellow. It should not be used in the case of EMERGENCY or if immediate attention is needed. In an Emergency, please call 911. If an urgent situation arises, some options are highlighted here: Get Help If You Need It (etsu.edu). Any concern about a trainee's welfare may be reported. This includes, medical, mental
health, academic, and physical health issues.
CARE reports can be confidential, but you are encouraged to provide information in case additional information is needed. The reporter’s identity will not be disclosed to the student of concern. A limited number of Internal Medicine faculty and staff have access to the form.
CARE reports are reviewed daily. If it is determined that further action is needed, the trainee will be contacted and interviewed in person to ensure safety. Depending on the severity of the concern, additional steps will be taken based on Quillen COM policy to include Fitness for Duty.
