  Undergraduate students may use this form to request a review of transfer courses to be used as General Education credit.

  Regalia - Permission Form
  Students may use this form to designate a third party to pick up regalia for them once the Jostens deadline has passed.

  Replacement Diploma
  Includes official digital copy. Legal name listed with ETSU will be used for diploma.

  Request for Commencement Invitation to International Guests
  Letters will be emailed to students. 

  Transferring Course in Last Term
  To provide documentation of a transfer course during the student's last term.
