Dr. Jay L. and LeVenia S. Allen Scholarship Award Criteria
The purpose of the Jay L. and LeVenia S. Allen Memorial Scholarship Award is to provide awards for meritorious students in Public Health and to create a lasting memorial to Jay L. and LeVenia S. Allen. Candidate for the award must:
- Be a rising Junior or Senior.
- Be a declared Public Health or Health Administration major.
- Have a recognized financial need.
- Have a minimum 3.0 GPA on all college work.

Ethan S. Bain Environmental Health Scholarship
To provide scholarship assistance to deserving undergraduate students enrolled in the Department of Environmental and Occupational Health and Safety Sciences or Department of Community and Behavioral Health in the College of Public Health at East Tennessee State University.
ELIGIBILITY
- Applications should be submitted to the ETSU College of Public Health Student Awards Committee by the prescribed application deadline.
- Entering freshmen as well as current full-time ETSU students, transfer students, or non-traditional students shall be eligible for the award per the below criteria.
- Once a student is selected to be a recipient, he or she may continue to receive the award to complete an undergraduate degree, not to exceed four consecutive years providing he or she continues to meet the below criteria.
CRITERIA
The Committee shall consider the following criteria in awarding scholarships.
Each applicant must:
- Be a declared Environmental Health major (1st priority)
- Be a declared Community and Behavioral Health major (2nd priority)
- Have a minimum 3.0 GPA for all college coursework.
To promote diversity at East Tennessee State University, preference for the scholarship is given to (in priority order):
- A Bahamian student.
- A Caribbean student.
The Committee shall also consider:
- Applicants should show strong potential for future leadership in their chosen profession and a commitment to returning to their home Nation.
- Applicants should exemplify the characteristics of care and concern for individuals and communities which guided Ethan S. Bain.
- Applicants should select their field placement site with consideration of the guiding principles and objectives of the Ethan S. Bain Environmental Health Foundation.
- Applicants should show an interest in mentorship and openness to provide guidance to future recipients of the scholarship.

The Thomas C. Fahres, RS, Environmental Health Scholarship Endowment
The purpose of the Thomas C. Fahres, RS, Environmental Health Scholarship Endowment is to provide financial assistance to deserving students in the Department of Environmental and Occupational Health and Safety Sciences in the College of Public Health at East Tennessee State University.
Thomas Fahres graduated cum laude from ETSU in 1978 with a Bachelor of Science in Environmental Health. He is a Registered Sanitarian. He served with the US Public Health Service Commissioned Corps, retiring with the rank of Captain. Assignments included the Indian Health Service, National Health Service Corps, and National Oceanic and Atmospheric Administration, US Department of Commerce. His other degrees are a Bachelor of Arts from the University of San Francisco and a Master of Science in Administration from Central Michigan University.
ELIGIBILITY
- Currently enrolled Junior or Senior students majoring in Environmental Health shall be eligible for the award per the below criteria.
- If a student is selected as a scholarship recipient for their junior or senior year they can apply and are eligible for a second year but are not guaranteed renewal. Awards do not have to be in consecutive years. No student can receive more than two awards.
CRITERIA
- Academic excellence as demonstrated by an overall GPA of 3.0 or higher and a GPA of 3.25 or higher in Environmental Health courses.
- Demonstrated financial need.
- Preference for students not eligible for in-state tuition.

Bruce A. and Amelia R. Goodrow Endowment
The purpose of the Bruce A. and Amelia R. Goodrow Endowment is to provide scholarships for ETSU students who are planning a career in public health.
Award Criteria:
- Preference will be given to candidates who are veterans of the military force of the United States residing in East Tennessee; to students who are residents of either Hawkins or Grainger County, Tennessee or from any other rural East Tennessee county, in that order.
- Candidates must demonstrate academic potential and ability, and be committed to a successful career in a public health profession. Preference will be given to candidates majoring in Community Health, Public Health Administration, Epidemiology, and Environmental Health.
- Candidates must meet all requirements for entrance into the university and into their chosen health major.
- The scholarship may be awarded to students on any classification level, but in order
to receive this scholarship in subsequent semesters, the scholarship holder must complete
at least 12 semester hours of course work each semester and must maintain the following
minimum GPA schedule:
- Freshman-3.0
- Sophomore-3.0
- Junior-3.25
- Senior-3.25;
- and Gradudate-3.5.
Qualified graduate students must also have a minimum of 2 years of documented real-world public health experience prior to applying.
Applications can be submitted by March 1 to the Office of the Dean for consideration by the College of Public Health Scholarship Committee.

Graduate Student Research Fund
The initial gift to establish the College of Public Health Graduate Student Research Fund was provided by Dr. Michelle Reece. The fund exists to provide financial assistance to ETSU College of Public Health graduate students seeking public health research experiences; including grant writing, research study design, research applications and management, data analysis, and presentation of research findings at local or national scientific meetings.
Examples of appropriate expenditures include but are not limited to: data collection, travel to and from the field/data collection site, research project supply and operational costs, and conference travel or registration fees. The fund may not be used for tuition or fees for courses or other school related expenses such as books.
To qualify for an award, students must submit a brief grant proposal. Proposals must be well written, clearly defined, feasible, and integral to the student's career development in public health and research training. In other words, the intellectual and scientific merit of the proposal must be clear.

John P. Lamb, Jr. Scholarship Criteria
John P. Lamb, Jr. Memorial Scholarship Endowment
Candidates for the award must:- Be a rising junior or senior.
- Be a declared Public Health major.
- Have a minimum 3.0 GPA on all college work.
- Have a potential to succeed in a public health career and
- Demonstrate leadership.

Dr. M.T. Morgan Scholarship
This scholarship honors the long and distinguished career of Dr. M.T. Morgan and provides funding to deserving students in the Department of Environmental and Occupational Health and Safety Sciences.
Criteria for eligibility:
1. Be majoring in Environmental Health.
Preference is given to students who (in priority order)
- Are members of the ETSU men's football team or the ETSU marching band;
Are residents of Northeast Tennessee (Carter, Cocke, Greene, Hamblen, Hancock, Jefferson, Johnson, Sevier, Sullivan, Unicoi and Washington Counties) or Madison County, North Carolina;- Are residents of other East Tennessee or Western North Carolina counties;
- Have a minimum 2.5 GPA on all previous high school and/or college work;
- Have documented financial need in the ETSU Office of Financial Aid.

Dr. Robert D. Patton Health Policy Internship and Community Health Scholarship Endowment
Each applicant to the Robert D. Patton Community Health Scholarship must:
- Be an undergraduate student majoring in Public Health with a Community Health concentration
Preference for the award will be given to:
- Qualified students from the state of Tennessee
Each applicant to the Robert D. Patton Health Policy Internship must:
- Be an undergraduate student majoring in Public Health with a Community Health concentration

Thomas L. and Jean L. Perry Jr. Scholarship
Thomas L. and Jean L. Perry Jr. have long ties to East Tennessee State University, the Health Sciences Department in the College of Public Health, and to the work of public education. The purpose of the scholarship is to provide financial assistance to deserving undergraduate students in the Department of Biomedical Health Sciences in the College of Public Health at East Tennessee State University.
Criteria for eligibility:
- Rising Junior or Senior majoring in Health Sciences in the College of Public Health
- Demonstrated financial need
- Academic excellence as demonstrated by a GPA of 3.0 or higher and/or other scholastic achievements
- Preference for students from Tennessee or Virginia

Payton and Angela Ward Pruett Public Health Scholarship Criteria
Payton and Angela Ward Pruett Public Health Scholarship Criteria
The purpose of this award is to provide scholarship assistance to deserving students enrolled in the College of Public Health at East Tennessee State University.
Candidates for the award must:- Be either an undergraduate or graduate Public Health student with an interest in environmental health sciences.
- Have demonstrated financial need.
- Have a minimum 3.2 grade point average (on a 4.0 scale) or other significant accomplishment
- Participate in at least one professional student organization

Kristen Taylor Spencer Memorial Scholarship
The purpose of this award is to provide financial assistance to academically eligible, and deserving undergraduate and MPH students in the College of Public Health including incoming high school seniors.
Criteria preferences for selection:
- Non-traditional student
- Demonstrated commitment to community service
- Demonstrated financial need
- GPA of 3.0 or higher
- Interest in living in and serving the immediate surrounding region during their career

Honors in Discipline
Focused on specific majors, students are eligible for scholarships awarded on a competitive basis. Scholarships for Tennessee residents cover full in-state tuition and most course fees for a maximum of eight semesters of study. Out-of-state scholarships cover the out-of-state portion of tuition. -
Joe F. Lynch Memorial Scholarship Endowment
Criteria for eligibility:
- Environmental Health major
- Demonstrate financial need
- Minimum GPA of 2.7 on previous high school or college work
- Resident of Carter, Washington, Blount, or Sevier Counties in TN
- First preference will go to children of employees of the State of TN, thereafter,all candidates meeting all other criteria will be considered.

The Dr. Vincent A. Sikora Memorial Fund in Environmental Health
Established: March 2004
Dr. Vincent A. Sikora III was a native of Laramie, Wyoming and was the son of Jean Walczyk Sikora and Vincent Sikora, Jr. Mr. Sikora was an attorney and an Associate Professor at East Tennessee State University in the Department of Environmental and Occupational Health and Safety Sciences. He was a member of St. Mary’s Catholic Church. He also served as a Captain in the United States Army. He was a kind and giving man who gave of himself to the community through various organizations. He coached in the Johnson City Major Little League and for the Johnson City Parks and Recreation. He was a member of several environmental health organizations and wrote a monthly Legal Briefs article for the National Environmental Health Association Magazine. He contributed to the advancement of the public and environmental health professor in many ways and he was committed to providing practical and relevant legal information for readers of the journal. In 2002, NEHA awarded the Harry Bliss Editor’s Award to Dr. Sikora, not only for his outstanding and popular column, but also for his contributions to the advancement of the cause and the interests of the association and its members.

State of Franklin Environmental Society Scholarship Endowment

- Gain an understanding of the State of Franklin Environmental Society through reading the history

Adkins Microbiology Scholarship
David M. Adkins, a 1990 graduate of ETSU, established this fund to honor and memorialize part and current professors in the Microbiology department. The fund provides financial assistance to deserving undergraduate students pursuing a degree in Health Sciences with a concentration in Microbiology.

Frist Global Health Scholars
In 2004, Senator Bill Frist established the nonprofit organization Hope Through Healing Hands. The purpose of Hope Through Healing Hands is to promote and increase quality of life for citizens and communities “using health as a currency for peace.” Hope Through Healing Hands addresses these issues by sponsoring global health programs including the Frist Global Health Leaders program. The Frist Global Health Leaders program provides students with the opportunity to travel to underserved countries around the globe and to improve the health of people living in those countries.
The Frist Global Health Leaders program may provide up to $2,500 per selected student. This amount cannot exceed 80% of the total expenses for the international field experience. Students will be responsible for the remaining 20% of total expenses.

The Wykoff Public Health Student Scholarship
In honor of his mother and father who inspired in him a passion and commitment to global health, Dean Randy Wykoff created the Wykoff Public Health Student Scholarship for international experiences. This fund is to be used to support international experiences for students pursuing a graduate degree in the College of Public Health by providing up to two $500 scholarships per year. The funds are to be used to help off-set international travel and related costs for graduate students participating in organized and approved field placement programs as part of their formal academic training. Preference is given to students from Tennessee who have had little or no international field work experience.

The Cal Garland Award for Internship Excellence
The purpose of the Calvin B. Garland Public Health Internship
Excellence award is to provide scholarship assistance to deserving students enrolled in the College of Public Health at East Tennessee State University who have shown excellence in their field placements.CRITERIA:
- Be a senior Public Health major in Community and Behavioral Health
- Excellent academic performance as indicated by a minimum grade point average of 3.0
- Actively participate in a student organization and/or have made a major contribution

The Dr. T.J. Wu Research Award
The College of Public Health recognizes one student per year in honor of Dr. T.J. Wu. The award is given for outstanding research accomplishments by a student in the College of Public Health. Nominations are submitted by Faculty Members in the College of Public Health and must include supporting comments that document the nominee’s achievements, including the significance of their research activities, funding record, publications, and presentations. Members of the college Scholarship Awards Committee reviews all nominations and determines the recipient of the award.

Scott C. Dulebohn Scholarship Endowment
The purpose of the Scott C. Dulebohn, MD Scholarship Endowment is to provide scholarship assistance to deserving students enrolled in one of the Academic Health Sciences colleges at East Tennessee State University.

Requirements:
- Entering freshmen/graduate students as well as current ETSU students, transfer students, or non-traditional students shall be eligible for the award.
- Once a student is selected to be a recipient, he or she may continue to receive the award to complete a degree, not to exceed four consecutive years providing he or she continues to meet the criteria.
- The applicants must be enrolled in one of the Academic Health Sciences colleges: Quillen College of Medicine, College of Nursing, Gatton College of Pharmacy, College of Health Sciences, or the College of Public Health.
- Recipient must have documented financial need, and must demonstrate a true passion for a career in healthcare.
- Priority will be given to students who have overcome adverse circumstances such as being raised in a single-parent household.
- Applicants must meet all requirements for regular admission to ETSU if new to the university.
- Applicants must be full-time students.

Dr. Scott C. Dulebohn is a practicing neurosurgeon in Johnson City. Scott’s approach to medicine is to treat patients as family members, as he believes mutual trust is a cornerstone of the patient-provider experience. Having grown up in the St. Louis, MO area, Scott and his wife, Heidi, were first attracted to Johnson City for both high-caliber fellow physicians, and quality of life. Scott’s early life was significantly impacted by a helping hand he received, which ultimately led him on the path to medicine, and his hope is to provide a similar experience to upcoming healthcare providers and practitioners. -
Eddie Williams, Jr. Memorial Rural Health Scholarship
Deadline: March 1
Requirements:
- Rising juniors or seniors with an interest in rural health issues
- Residents of northeast Tennessee, southwest Virginia or western North Carolina
- A 500-word essay describing applicants interest in rural health issues is required (contact the scholarship office for more information)
- Minimum GPA 2.5
- Demonstrated financial need
- Recipients will be required to make at least one public presentation about their work/interest in rural health while receiving this scholarship

Dr. Charlena M. Seymour Scholarship
This scholarship was established to recognize women pursuing an advanced degree in the fields of Public and Environmental Health. The Scholarship is named in honor of Charlena Seymour whose professional accomplishments and vision, leadership, and compassion for her fellow man make her an outstanding role-model to women.
$1000 will be awarded. The winner will be notified by September 1st. A formal announcement of the winner will be made at the AEHS Foundation Annual International Conference on Soils, Sediments, Water, & Energy in October.

Dr. David F. Ludwig Memorial Travel Scholarship
The AEHS Foundation is pleased to offer the David F. Ludwig Memorial Student Travel Award. This scholarship was established to recognize students pursuing research in Ecology and Ecological Sciences. Dr. Ludwig’s passion for science, travel, education, and exploration are the inspiration for this award, and the AEHS Foundation is pleased to do our part to carry on his legacy. This award will be made annually at the beginning of the fall academic semester for travel costs associated with research in the field of ecology, including travel costs to present the results of ecology-related research, or conference attendance to present research or for professional development. Preference will be given to those conducting/presenting research.
Up to $1000 per student will be awarded. Winner(s) will be notified by September 1st. A formal announcement of the winner(s) will be made at the AEHS Foundation Annual International Conference on Soils, Sediments, Water, & Energy in October.
