Will there be a copay for an office visit?
Yes there is for our faculty/staff members. A copay will be made prior to seeing one of our medical care providers. The amount varies depending on the insurance plan.
For students, there is no copay. If you a currently enrolled student there is no charge for an office visit. -
Where can I park when I visit the Health Center?
There are 3 designated clinic patient parking spots in Faculty/Staff Lot 35 located behind Nicks Hall. -
Do I need to have a permit to park in the ETSU clinic patient reserved spaces?
Yes, you will need a Visitor's permit to park in those reserved spaces if you do not have a current ETSU student or faculty parking decal on your vehicle. To request a Visitor's permit, visit the Parking Services Website. -
I received a parking citation in the ETSU clinic reserved spaces, what do I do now?
You may receive a citation for being parked in the reserved spots. If this happens, bring the citation to the University Health Center for validation if you had an appointment at the Health Center.
If you have already left the campus, you may bring your ticket back to the Health Center at your earliest convenience

Where do I send my Immunization Record?
You may either hand in your documentation in person, fax them to 423-439-4560, mail them to the University Health Center Box 70675, East Tennessee State University, Johnson City, TN 37614 or submit them directly to our Immunization Coordinator, Megan Flora, at immunizations@etsu.edu. -
Am I allowed to take a photo of the required documents and email them to the Immunization Coordinator?
Yes, you may. When taking photos of the necessary documents make sure we are able to see the top, bottom and both sides of the paper. For example, if your immunizations are listed in the top half of the document and the lower half is blank, do not zoom in and take a photo of the top half only. All sides of the document must be seen in the photo in order to accept it. If the photo does not have these elements, we will ask for you to resubmit the immunization records. -

Is there a required document for listing immunizations?
Yes there is! Print out the ETSU Immunization Form and have your medical provider complete it. -
Do I need to use the ETSU Immunization Form? Can I have the Health Department or my medical provider print off a copy of my immunization records?
You may have the Health Department or your medical provider print out a copy of your immunization records. However, it is your responsibility to make sure the documentation has the required elements listed below:
- Facility Letterhead/Facility Stamp
- Patient name and Date of Birth
- Medical Professional signature
- Two doses of required vaccines (Varicella and MMR) given after 1 year of age and at
least 28 days apart,
- Or a titer quantitative lab report(serology proof, IgG antibodies of immunity of diseases),

Will you accept the blue or yellow immunization booklets?
No, we are unable to accept these booklets. All immunizations must be medically documented from your medical provider of health department on an official state-approved immunization form or on the ETSU Immunization Form. -
I had the chicken pox as a kid. Do I still need to provide immunization records for the varicella vaccine?
No, you do not need to provide documentation you have had the varicella vaccine. You will need to provide documentation from the Health Department or your medical provider verifying you have had the disease. If you are unable to obtain this documentation, a titer quantitative lab report illustrating your immunity will be sufficient.

I had a titer drawn to show immunity to varicella, measles, mumps, rubella, ect. Will this satisfy my immunization requirements?
Documentation of blood test (titer-serology) showing immunity to measles, mumps, and rubella is acceptable, however, if any one of the three is negative, 2 doses of vaccine must be provided to University Health Center. This documentation is acceptable showing varicella immunity as well. -
Do I need to submit the Hepatitis B/Meningitis Immunization History Form if I already have documentation showing I received the Hepatitis B and Meningitis vaccines?
If you are under 18, you will still need to have a parent/guarding complete and submit this form regardless of proof showing you received the Hepatitis B and Meningitis vaccines. You cannot register without this form on file.
This form is for legal purposes only. Completion of this form does not constitute submission or refusal of receiving required vaccinations. This for does not exempt you from the Meningococcal vaccine if you are under 22 and plan on living on campus. -
I will only be taking online classes. Do I still need to turn in my Immunization Records?
If you are an online only undergraduate student or graduate student registering for ≤11.99 credit hours or ≤8.99 credit hours respectively, you will not have a registration hold placed on your account. However, if you will be a full-time online only student (> 11.99 credit hours or >8.99 credit hours) you will need to complete and submit the Registrar's Online Only Immunization Waiver.
Keep in mind, this waiver will need to be completed and submitted prior to your registration date for every semester you plan on being a full-time online only student. This is NOT a one-time submission waiver encompassing the entire school year.

How do I get international student insurance?
The international student insurance provider is currently ISP International Student. Please contact the International Office at 423-439-7737 if you have any further questions. -
Do you accept immunization documents in languages other than English?
No, all immunization records must be documented properly in English only. -
I'm an international student, do I have to get a Tuberculosis (TB) test at the University Health Center?
Yes, we ask that you complete a TB test with the University Health Center. We are located in Roy S. Nicks Hall suite 160 and our phone number is 423-439-4225. Please call to schedule an appointment.
